Job Description

Provide supportive counseling, independent living skills training, and case management for individuals residing in a community treatment facility. Core responsibilities include assisting residents with daily living tasks, monitoring medication, providing crisis intervention, managing transportation needs, and supporting day-to-day residence operations (including serving as the point of contact in the absence of the Senior Counselor).

Job Responsibilities & Requirements

Key Responsibilities:

  • Independent Living Skills & Counseling: Train and guide residents in activities of daily living (ADLs); deliver supportive counseling, behavioral interventions, and crisis management; monitor medication adherence.
  • Care Planning & Case Management: Participate in individual service planning and evaluation; write timely monthly progress notes; provide case management, resource linkages, and follow-up care.
  • Transportation & Outings: Accompany residents and provide transportation for medical appointments, personal shopping, recreational outings, and community integration.
  • Facility Operations & Emergency Support: Assist with routine facility management; step in for the Senior Counselor when needed; report medical or psychiatric emergencies to leadership in accordance with agency protocols.

Requirements:

  • Education & Experience:
    • Bachelor’s degree in a Human Services field OR
    • High School Diploma/GED paired with minimum five (5) years of experience in human services.
  • Licensing & Vehicle Requirements: Active, valid Driver’s License, access to a reliable personal vehicle, and proof of current auto insurance.
  • Clearances & Compliance: Must successfully pass fingerprinting and a criminal background check; adhere strictly to HIPAA compliance standards and attend all mandatory agency trainings.
  • Core Competencies: Strong critical thinking, crisis intervention, organizational, and interpersonal communication skills.
