Job Description
Provide direct personal care, companionship, and daily living support for individuals and families experiencing incapacitation, chronic stress, or family disruption. Core duties include assisting with household management, coordinating medical care, facilitating life skills education, managing client documentation, and providing transportation for community appointments.
Job Responsibilities & Requirements
Core Responsibilities:
- Personal Care & Medical Support: Assist clients with personal hygiene, grooming, and clothing selection; schedule appointments, accompany clients to medical visits, monitor medication schedules, and report changes in condition to supervising case managers.
- Household Management & Meal Preparation: Perform essential housecleaning (vacuuming, sweeping, dusting, kitchen/bathroom sanitation); assist with budgeting, bill paying, and grocery shopping; plan and prepare nutritious meals, including prescribed dietary restrictions.
- Independent Living Skills & Education: Instruct clients in basic housecleaning, laundry, meal planning, and budgeting; assist with applications for financial, food, and medical assistance programs; support educational group lectures.
- Client Advocacy & Crisis Support: Provide emotional support to families under chronic stress; assist with service plan development; maintain tickler files for review deadlines; participate in multidisciplinary case reviews.
- Transportation & Community Access: Safely transport clients to appointments, community resources, and recreational activities using proper vehicle safety guidelines.
Requirements:
- Education & Training: High School Diploma or GED required; willingness to participate in ongoing continuing education and unit staff meetings.
- Skills & Qualifications: Valid driver's license with an acceptable driving record; strong interpersonal, conflict-resolution, and listening skills; basic computer literacy for electronic recordkeeping.
- Competencies: Knowledge of confidential record management, simplified budgeting, and community assistance resources; ability to support adults facing severe mental health challenges, disabilities, or life transitions.
