Job Description

Provide comprehensive addiction treatment, therapeutic counseling, and case management services for patients recovering from substance use disorders and co-occurring behavioral health conditions. Operating within a structured treatment environment, the Counselor II conducts initial and ongoing clinical assessments, formulates individualized care plans, facilitates group and family therapy, provides crisis intervention, and coordinates aftercare planning to support long-term recovery.

Job Responsibilities & Requirements

Key Responsibilities:

  • Clinical Assessment & Care Planning: Conduct initial and follow-up diagnostic assessments; build, review, and adjust individualized treatment plans; evaluate patient needs for specialized referrals or external program placement.
  • Counseling & Education: Facilitate or co-facilitate individual, group, and family therapy sessions; deliver psychoeducational workshops on substance use and mental health for patients, families, and community groups.
  • Case Management & Liaison: Serve as primary case manager for assigned caseload; act as a liaison between patients, referral sources, self-help groups, Employee Assistance Programs (EAPs), and insurance carriers.
  • Crisis Response & Aftercare: Deliver timely crisis intervention as needed; develop structured aftercare and continuing care plans to assist patients in maintaining recovery milestones.
  • Documentation & Quality: Maintain accurate, timely, and concise clinical documentation in accordance with hospital standards, accreditation guidelines, and state regulations.

Requirements:

  • Education & Experience (Must meet one of the following combinations):
    • Bachelor’s degree in a social/health services field + 2+ years of counseling experience OR
    • Bachelor’s degree in an unrelated field + 4+ years of counseling experience OR
    • Master’s degree in a social/health services field + 1+ year of professional work experience.

      (All degrees must be from an accredited college or university).

  • Certifications: CPR and de-escalation certifications required (training provided on-site upon hire); First Aid certification as required by facility standards.
  • Licensure: State counseling licensure or CADC/SUD credential as required by Georgia regulatory standards.
  • Core Competencies: Proven skill in group therapy, crisis intervention, family engagement, multi-disciplinary case management, and clear clinical charting.
