Job Description
As a Mental Health Court, Case manager, your responsibilities would include:
- Assess clients being referred to 19th Judicial Circuit Mental Health Court Project.
- Completes comprehensive needs assessments to assigned clients in mental health court for purposes of identifying needs and services. This includes providing outreach services to help identify clients in need of substance abuse services.
- Acts a primary Mental Health Court contact for New Horizons clients participating in Indian River Mental Health Court.
- Coordinates with other New Horizons staff to assist clients within Mental Health Court to access needed services including mental health and substance abuse services
- Provide assistance to clients for the purpose of accessing needed services
- Participates in committees, meetings, clinical training, staff development and community relation activities.
- Prepares written documentation for each service conducted.
Job Responsibilities & Requirements
New Horizons will need you to have the following qualifications:
- Bachelor's Degree from an accredited university or college with a major in the field of counseling, social work, psychology, nursing, rehabilitation, special education, health education, or a related human services field
- One year experience
