Job Description

Provide community-based case management and rehabilitative support to youth enrolled in Comprehensive Community Services (CCS) and Children’s Long-Term Support (CLTS) programs in Lincoln County. Working in partnership with clients, families, treatment teams, and community providers, this full-time role assesses needs, facilitates service planning, delivers skill teaching, and coordinates care to foster recovery, independence, and safety.

Job Responsibilities & Requirements

Key Responsibilities:

  • Assessment & Service Planning: Conduct initial and ongoing psychosocial assessments; determine program eligibility using state-approved functional screens; develop and update individualized service/treatment plans.
  • Care Coordination & Advocacy: Coordinate services across psychiatry, primary care, schools, courts, housing, and substance use providers; facilitate multidisciplinary team meetings; advocate for medical, educational, and community resources.
  • Rehabilitative Skill Teaching & Crisis Support: Provide side-by-side skill instruction (daily living, budgeting, symptom management, social skills); assess risk factors and coordinate with crisis services to establish safety and stabilization plans.
  • Community Integration & Documentation: Conduct facility visits for discharge planning; assist clients with vocational and educational readiness; maintain thorough, timely documentation (progress notes, referrals, treatment plans) in compliance with state regulations.

Requirements:

  • Education: Bachelor’s degree in Human Services, Social Work, or a closely related field required (Master’s degree preferred).
  • Experience: Minimum 1 year of direct case management experience required (3+ years of case management and 1+ year of experience in CCS, CSP, CLTS, or CST preferred).
  • Licensing & Travel: Active Wisconsin driver’s license with an unrestricted driving record, approval to transport clients, and access to a personal vehicle required.
