The Behavioral Health Care Manager is a core member of the Collaborative Care Management team, working closely with primary medical providers, consulting Psychiatric Consultants, and broader healthcare staff. This role supports and coordinates integrated behavioral and physical healthcare for an assigned patient caseload. Primary responsibilities include patient engagement, behavioral health screening, brief evidence-based interventions, treatment monitoring, care coordination, and registry management.

Job Responsibilities & Requirements
Primary Responsibilities
- Patient Assessment & Engagement: Screens and assesses patients for mental health and substance use disorders; educates patients on treatment options; builds strong therapeutic relationships to drive care engagement.
- Collaborative Care Coordination: Coordinates physical and behavioral healthcare with medical providers and Psychiatric Consultants; participates in caseload consultations; facilitates treatment plan updates for non-responding patients; links patients to higher levels of care as clinically indicated.
- Brief Behavioral Interventions: Delivers brief, evidence-based interventions (e.g., Motivational Interviewing, Behavioral Activation, Problem-Solving Treatment); assists patients in developing self-management and relapse-prevention strategies.
- Medication & Outcome Monitoring: Monitors medication adherence, clinical progress, side effects, and symptom changes across in-person, phone, and virtual touchpoints; re-engages patients who miss follow-up care.
- Referrals & Community Navigation: Facilitates internal and external referrals for specialty mental health care, substance use treatment, housing assistance, vocational support, and community-based resources.
- Documentation & Registry Management: Maintains accurate, timely documentation in the Electronic Health Record (EHR); tracks patient follow-ups, outcomes, and care communication using the Collaborative Care patient registry.
Requirements & Qualifications
- Licensure / Education: Must possess an active professional license as an LCSW, LSW, LCPC, LPC, LMFT, RN (BSN preferred), NP, or Licensed Clinical Psychologist.
- Preferred Experience:
- Background in screening, assessing, or treating common mental health and substance use disorders.
- Prior experience in primary care integration, behavioral health, or the Collaborative Care Model (CoCM).
- Knowledge of brief evidence-based interventions and basic psychopharmacology within license scope.
- Core Competencies:
- Strong interdisciplinary collaboration, organization, and follow-up capabilities.
- Exceptional written, verbal, and therapeutic communication skills.
- Proficiency in EHR documentation and working across in-person, telephone, and telehealth settings.
